Our Network
Eliot provides Community Based Flexible Supports to adults with mental illness, who are referred by the Department of Mental Health in the Beverly, Lowell, Lynn, Metro North and Metro Suburban areas.
Our Approach
For many years Eliot has supported consumers in maintaining community based housing as well as identifying and linking consumers to a wide range of community resources that foster choice and the development of natural supports. Our services are flexible and individualized, allowing for increased supports and services during times of increased acuity and collaboration with other service providers, community and natural supports based on consumer needs and preferences. We believe that the integration of community based services, natural supports and other service providers is a crucial component of providing recovery based services.
We recognize the unique needs of each consumer, in an environment that promotes independence not dependence; and encourage individuals to investigate service options in a manner which empowers them to choose services that meet their needs.
Our Services
Community Based Flexible Supports (CBFS) provide an array of rehabilitative treatment options that promote consumer choice and facilitate recovery. Services include a range of community based housing options, interventions and supportive services to manage psychiatric symptoms, promote wellness and the management of medical conditions and restore/maintain daily living, social and vocational skills. Services are designed to maximize flexibility, strengthen consumer driven care and treatment planning, integrate peer workers and encourage person focused outcomes.
Each consumer works with CBFS staff, community treatment providers and natural supports to develop an Individualized Action Plan (IAP), which is a comprehensive plan of all services/supports the consumer wishes to receive. The IAP process is person-centered, strengths based and recovery driven. A consumer's strengths, experiences, culture, values, preferences, needs, hopes and life goals guide the type, intensity and duration of services provided.
