Program for Assertive Community Treatment (PACT) is designed to deliver an intensive community based array of services for individuals with mental illness who require a higher level of support, but may be resistant to traditional models of treatment. Using a team of highly trained, multi-disciplinary professionals, the PACT service is available 24/7 and does the majority of its work with individuals at home, at work or in the community through intensive and diligent outreach strategies.

Services provided through the PACT Team include:

Medication management support
Employment stabilization and support
Health and wellness education
Substance abuse counseling

The evidence-based approach PACT operates with integrates clinical, behavioral, educational, and rehabilitative supports, with goals of less frequent hospitalizations, improved medication adherence, and stronger community functioning.

Our teams are comprised of clinicians and behavioral health professionals with expertise in Rehabilitation, Counseling, Nursing, Psychiatry, and Social Work.  Staffing standards and intervention protocols are designed to meet PACT national fidelity standards endorsed by the National Alliance for the Mentally Ill.

Eliot operates PACT Teams in the Lowell, North Shore and Metro North-Lynn areas.  Staff are available 24 hours a day, 365 days a year.   Referrals to the PACT program are through Massachusetts Department of Mental Health.
