When Kevin (name changed for confidentiality) joined Eliot’s Behavioral Health Community Partners (BH CP) program in October 2023, he faced more than a health crisis. He was experiencing homelessness, struggling with financial hardship, and unable to maintain consistent medical care. These challenges—known as social determinants of health (SDOH)—were immediate obstacles to his recovery. Eliot’s BH CP team took swift, decisive action to address these barriers, laying the groundwork for lasting stability and improved well-being.
Kevin’s Journey: Turning Barriers into Opportunities
At just 25 years old, Kevin found himself hospitalized for psychosis and issues related to medication adherence. With no stable place to live, he was unhoused living in a car. Recognizing that housing instability could derail his recovery, Kevin’s Care Manager met him during his inpatient stay to create a personalized, actionable care plan. Within days, the staff coordinated Kevin’s transition to the Eliot Respite Program, providing him with a secure, supportive environment to stabilize and manage his new medication regimen.
This was just the first hurdle. Kevin needed immediate access to basic resources to rebuild his life. His Case Manager addressed these needs expeditiously, including:
- Personally accompanying Kevin to the Social Security office, ensuring he successfully applied for benefits.
- Guiding him through the SNAP (food assistance) application process, which was approved, giving him consistent access to nutritious food.
- Assisting with Department of Transitional Assistance (DTA) resources to secure short-term financial support.
- Scheduling and attending primary care appointments with him to prevent gaps in medical care.
- Helping him complete applications for and secure permanent housing through the Department of Mental Health (DMH).
Each resolved issue propelled Kevin to greater independence. With secure housing in place, Kevin began focusing on employment, a key goal in his recovery journey.

“I feel a lot better about my future and know what’s coming next,” Kevin shared. “Having a place to live means I can now focus on finding a job—that’s what I really want.”
