Early Episode Psychosis (First Episode) is a specialized, evidence-based program treating individuals under age 35 and their families who have recently begun to experience psychosis related symptoms.

Our program is designed for individuals who meet these core criteria (with some flexibility as we triage and review each referral):

  • Timing: First episode(s) occurred within the past 3 years

  • Clinical presentation: Psychosis is a clear and present concern, even if the diagnosis is still evolving

  • Age range: 13 to 35

In early episodes, a clear diagnosis often isn’t established yet—so our approach is designed to evaluate, stabilize, and coordinate care while symptoms and needs are better understood. Individuals accessing the program are likely to be diagnosed with prodromal psychosis, schizophrenia, schizophreniform disorder, schizoaffective disorder, or paranoid delusional disorder.

Please note: we do not enroll individuals with medical- or substance-induced psychosis.

What to Expect

This is an intensive, team-based program, meaning multiple providers and service types may be involved—especially early on. We work with the individual (and family, when appropriate) to evaluate what services are most helpful and realistic, including how to navigate any supports already in place.

In cases where symptoms are severe and there is diagnostic certainty, we may proceed with conditional enrollment for up to six months while completing assessment and determining longer-term needs. If the presentation is ultimately not a primary psychotic disorder, we ensure stability and then transition the individual to an Eliot CBHC treatment team or other appropriate community-based care.

Care coordination matters—especially medication. Because medication treatment can be complex in early psychosis and often requires close team communication, we strongly recommend transitioning medication management to the program’s prescriber for coordinated care. In some cases, an outside individual therapist may remain involved if it makes sense clinically and logistically.
