Eliot’s Program for Assertive Community Treatment (PACT) offers services to individuals with mental illness who require a higher level of support, but may be resistant to traditional models of treatment. A team of clinicians and behavioral health professionals with expertise in Rehabilitation, Social Work, Counseling, Nursing, and Psychiatry work with individuals at home, at work, or in the community.
PACT services include:
- Comprehensive needs assessment
- Vocational assistance and support
- Housing and entitlement assistance
- Medication assessment/prescribing
- Safety and risk assessment/planning
- Linkage to health and mental health services
- Discharge coordination from inpatient facilities
- Forensic assessment and consultation
- Support and assistance with daily living skills
- Substance use, support and service linkage
- Transportation training and support
- Peer Support Services
